* Proof of Residency - Examples include: monthly utility bill, rental agreement, rental receipt from receipt book complete with address, etc. These things will not be accepted as proof of residence: cell phone bills, driver's license, vehicle registration, hand written rental receipts not from a receipt book, etc.

 

* The last report card from the previous school (This is if you are registering your child for the first time at any school this 2023-24 school year.)

 

* Withdrawal form (This is if your child started this 2023-24 school year at another school.)

 

* If coming from a different state, we will need shot records on a Georgia form and Eye, Ear, and Dental records on a Georgia form.
